Minnesota Correctional Facility Red Wing Death




The investigation into the death of a juvenile offender, who was found dead last October  19th at the Red Wing Correctional Facility, has been completed.


The Minnesota Department of Corrections (DOC) says the Southern Minnesota Medical Examiner’s Office’s autopsy report said the cause of death was a self hanging and the DOC’s investigation concluded the correctional facility violated four correctional standards.


The report states that the facility was supposed to remove a staff person from their position in June of 2023, because they didn’t  cooperate with background check studies.


The investigation reports staff member was working during the incident last October.


The report concludes the facility failed to complete an on-time well-being check on October 18, 2024, with the check occurring 11 minutes late.


Also, camera footage showed the checks lasted approximately one second, not allowing enough time to ensure the safety and well-being of residents.


The investigation uncovered another pair of violations.


The time frame of 1:26 p.m. to 2:24 p.m. of October 18th during which the juvenile was unaccounted for with no documentation as to where they were and the facility is required to document attempts to contact the child’s parent or guardian to seek permission for administering medication.


There was no documented attempt to seek permission to administer medication.
